The CDHB have funded a free service that can be accessed through your local pharmacy to help you manage your medication. It involves a consultation with an accredited pharmacist and can take place at your local pharmacy or in your own home. 81 of the 109 pharmacies in Canterbury have an accredited pharmacist. It is especially helpful for people who have multiple conditions and a complex list of medication. Also targeted to people who have difficulty remembering to take medicines and those with asthma, diabetes, COPD, or heart disease.
Statistics show that 30% of acute medical admissions to hospital for people over 65 are medication related. Over time it is hoped that this can be reduced with good management. It may also help prevent waste of medications.
