Partnership Community Workers (PCWs) are available to support older people access health care including assisting them to find solutions to transport problems. They also work with other social services agencies to provide a co-ordinated response to the person’s needs.
Referrals are made through the GP’s or Practice nurse, or directly to the appropriate PCW.
